DOCUMENTS
To be sent to the client 3 days before (or on booking) the session. To be compliant with the Act a Consent Form must be completed and returned prior to the 1st session commencing.
SDH is governed by the
General Health Services Act – Code of Conduct.
It’s imperative you familiarise yourself with The Code and a copy must be given to your client on email along with the Consent Form or handed to them in person if it’s an in-person session. You must also have a link to it on your website, if you have a website.
